Your data, kept to the minimum.
Demo text for the Assay template. It is not legal advice: replace every section with wording reviewed for your own clinic and country.
Who we are
Assay Clinic Ltd runs the preventive health clinic at 18 Wigmore Street, London W1U 2RF. We are the data controller for the personal information described in this policy.
This policy covers the clinic, this website, the Assessment and the membership.
What we collect
Your contact details, your medical history, the results of every test and scan, and the notes your physician writes. We collect only what we need to run your Assessment and membership.
If you use the visit form on this website, we receive your name, your email and the evening you asked for.
How we use it
To carry out your Assessment, write your report and plan, and follow up with you as a member. Health data is a special category of data: we process it to provide health care under the responsibility of a registered clinician (UK GDPR Article 9(2)(h)).
To take payment, keep our accounts and answer your messages.
How we keep it
Your records are encrypted and stored in the UK. Only the clinicians caring for you and our records manager can open them.
We keep clinical records for as long as the law requires for adult health records, and visit requests that never became an appointment for 12 months.
Your rights
Under UK GDPR you can see, download, correct or delete your records at any time, except the clinical records we must keep by law. Write to us at the email address at the bottom of every page and we reply within one month.
If you are not happy with our answer, you can complain to the Information Commissioner’s Office (ICO).
